$275.00 US Club Registration and Membership Fee (per player)
$120.00 Club Membership and Administrative Fee (Due Sept. 1) per player
This form combines the player’s medical treatment authorization/liability waiver, and a confirmation the player is registering with your club. This form must be retained for at least five (5) years or until the player’s 18th birthday, whichever occurs last. This form is required and registrars may certify they are in possession of it when registering with US Club Soccer. The club/org will retain this form unless requested by US Club Soccer.
Deadline for Scholarship Application is June 15th.
To be eligible and considered for a scholarship, application, and all necessary paperwork needs to be completed and mailed in by June 15th.
Sorry, we are not accepting emails for scholarships. All applications need to be addressed to Scholarship and mail to the office. Applications have to be delivered to the office by June 15th, or else you may not get what you need. We have a limit to how many scholarships we can offer. When we run out, we are out, it will be up to each team to organize a fundraiser to help out those that need help on the team.
Approval of a scholarship mandates the player is committed to the club for the entire seasonal year, will participate in all games, practices, scrimmages, and team activities, unless school or religious events conflict. Additionally, the player will participate in all team fundraisers. Our monthly training fees are a one time fee, that we give our families the option to payout over 12 months. Player will owe the full amount of the scholarship if player quits, or seeks a release from the club before the seasonal year is complete.
